I have BCBS of RI and recently rec'd a letter from them with an offer of $175 in free Visa giftcards. You basically must spend about 1/2 hour on the phone with them 3-4 times discussing your current health. I did it and am getting my first $100 Visa Card. It's based out of their Care Coordination Program.
Worth contacting them if you carry their insurance and see if they have the program in your state.
