3. CUT out the UPC Symbol from your package of Aquafresh White Trays;
4. CIRCLE your Aquafresh White Trays purchase on your register receipt, dated between 7/23/07 -9/30/08;
5. GO to http://www.bella.com/whitetrays to view the available Bella t-shirts.
6. CLICK "Registration Form" on the http://www.aquafresh.com/whitetrays/bella page, print out and complete the Mail-in
Form, which includes the Bella t-shirt order form;
7. STAPLE your Aquafresh UPC Code and your original register receipt, dated between 7/23/07 – 9/30/08, to your completed Mail-in Form, and
8. MAIL your Registration Submission, in a properly stamped envelope, postmarked by 10/7/08 and received by 10/14/08, to: AQUAFRESH WHITE TRAYS BELLA T-SHIRT OFFERBella T-shirt Program P.O. Box 1325 Grand Rapids MN 55745-1325
