How to receive your Savings Coupons: 1) Purchase five (5) participating products (Vlasic®, Open Pit®, Armour®, Duncan Hines®, Log Cabin® or Mrs. Butterworth’s®) between 4/5/09 and 5/5/09; 2) Download this order form and hand print your complete name, mailing address and e-mail address; and 3) Mail your completed order form, the original cash register/purchase receipt(s) dated between 4/5/09 and 5/5/09 with the participating products circled and the corresponding UPC’s from five (5) participating products in an envelope to: Great Taste, Big Savings Offer, P.O. Box 2855, West Caldwell, NJ 07006-2855.
All coupon requests must be postmarked by 6/5/09 and received by 6/19/09. Limit six (6) Savings coupons per household. Cash register/purchase receipts dated before 4/5/09 and after 5/5/09 will not be honored.
