2. Once you are Signed in, Click here and select "Start Repair Service"
3. Click "Start Search" for Nearest Repair Location
4. On next page, select "Phone" for which Device is being repaired
5. Follow On-Screen Prompts to Select your eligible device (list of eligible devices below)
6. On next page, enter your zip code then Select your Nearest UBREAKIFIX Repair Shop
7. Click "See All" to Select Appointment Time for Repair then Click Confirm
8. Confirm Cracked Screen as Issue then on following page, enter your Phone IMEI Number then click Register. Enter your contact info then click "Submit Service Request"

Eligible Devices:

• Galaxy S series device that is Galaxy S8 model year or later
• Galaxy Note series device that is Galaxy Note8 model year or later

Excludes:

• Galaxy Fold
• Galaxy Z Flip
• Galaxy Z Fold2
• All A & J models
• All Galaxy S and Galaxy Note models (prior to S8)
